Verrucous carcinoma. Image 3 looks like BXO. Is the biopsy from the penis?

Agree: verrucous carcinoma associated with lichen sclerosus.

Hybrid penile carcinoma, mixed verrucous and warty (condylomatous) type. Lichen sclerosus is associated to the verrucous type and HPV-induced changes are associated to the warty (condylomatous) type.

Yes we reported this as verrucous carcinoma arising in lichen sclerosus / BXO (foreskin). I did not think there were any features to suggest HPV (I think the changes here are non-specific) and I believe p16 was negative too.
