I thought Ichthyosis vulgaris...but from birth?

on the other hand i don't see any granular layer

Agree with Ichthyosis, not vulgaris but probably X linked due to the basal vacuolar degeneration and hyperkeratosis of folicular and sweat duct orifices. Fits better with the clinical (male patient lesions present since birth)

Agree with ichthyosis.  I cannot differentiate, on microscopy, between ichthyosis vulgaris, lamellar ichthyosis and X-linked ichthyosis.

The hyperkeratosis along with the hypogranulosis makes me think in Ichthyosis vulgaris too.

The absent stratum granulosum is striking, fits with ichthyosis vulgaris

The pictures I took don't show the granular layer, but it is present.  So sorry about that!  This was reportedly a case of lamellar ichthyosis.
