I think these structures within the shaft are bubbles. Mainly in the region of the medulla. I think this could be "bubble hair", a defect that results from over heating (cosmetic effect)... Have never seen it, so I might be totally wrong :-)

Well done, Mona!

This is an example of ''bubble hair'', an acquired hair shaft abnormality. This lady did have a history of both chemical treatment and overheating. The damaged hair was trimmed and the new hair growth has been normal ( she has now altered her hair styling techniques!)  

There is only limited histo description of this entity. However, there is a lovely histology image of this diagnosis  (rather similar to this case) and some EM images as well in Sperling's 'Atlas of Hair Pathology', a book I would highly recommend for anyone reporting alopecia biopises.

Pili annulati is a good differential, but there was no alternating pattern of light and dark bands clinically or histologically. (the bubbles were randomly scattered in the shaft).
