I think this is neurofibroma (possibly plexiform, i.e. neurofibromatosis), associated with a vascular malformation. It has been reported before in five cases in this paper:-

J Vasc Surg 2007 46(3) 475-484.

I also thought of spindle cell hemangioma, but the vessels are not cavernous and there are no blister cells. Could we have an S100, NFP, some endothelial markers and perhaps a tol blue or CD117?

This lesion has overlap with angioleyomioma (mainly because of the thick blood vessels) and myopericytoma (perivascular cells and vessels without myoid appearence). This overlap is seen sometimes, if I had to choose I would favour myopericitoma but in real life I would diagnose it as a lesion with hybrid features.

I think the spindle cells will prove to be myogenic and, therefore, I will call this an angioleiomyoma.

I too had in mind myopericytoma / angioleiomyoma group of perivascular cell tumours.

Yes, nice overlap of myopericytoma and angioleiomyoma like areas...highlights the close relationship in these 2 entities.
