(Juvenile) xanthogranuloma  is a nice suggestion. Another entity to consider is cellular neurothekeoma, in which Touton type giant cells may be ocasionally present. In this case, the absence of eosinophils and the subtle cellular compartimentalization make me think of cellular neurothekeoma. It is true that eosinophils may be completely absent in early lesions of xanthogranuloma but in this case the presence of Touton type giant cells would indicate this is not such an early lesion.

Weedon states that juvenile xanthogranuloma should be called ''adult xanthogranuloma'' or plain and simple ''xhanthogranuloma'' when the onset occurs in adolescence or adult life which I think it is logical

Considering d/d of juvenile xanthogranuloma and xanthelesma. Although site more typical of latter, the deep  nature of the infiltrate and presence of Touton type giant cells favour JXG. Important to check for serum lipid levels.
