Yes, dysplastic nevus. There can be variability in how these are graded. A few years ago a survey was conducted at the LDS and the response from the audience varied from benign nevus to melanoma. My approach to these lesions is to recommend a re excision for lesions that are of moderate to severe architectural and cytologic atypia. The older the patient, the more concerned I tend to be. I also have a low threshold for re excision of lesions that look like this, but are on the face of an older patient (I tend to use 60 years of age as a cut off).
