Looks like a lip biopsy with granulomatous inflammation. Differential diagnosis consists of cheilitis granulomatosa as a manifestation of Melkerson Rosenthal syndrome, foreign body granuloma due to material injected for lip augmentation and sarcoidosis.

Agree completely. CPC is needed and I go mostly with cheilitis granulomatosa.
