Sincerely it's difficult for me, but Vulvar Aggressive Angiomyxoma could be a good spot.

Young female reproductive years, superficial lesion, hypo/ hyper cellular areas with rich vascular component. plump to spindled cells clustered around blood vessels. all these findings are suggestive of angiomyofibroblastoma. usually desmin positive.  most of the aggressive angiomyxoma is deep infiltrating lesion in older women. no such plumped spindle cells clustered around vessels. so not to me here..

Aggressive Angiomyxoma

Hi everyone! This is a tough one. I never gave you the size of the lesion, which is always so helpful. This is it; imaging studies showed no residual tumor. We also considered the possibility of a decidualized angiofibroma or fibroepithelial polyp. Desmin is negative, which argues against angiomyofibroblastoma.

It's a very small tumor (about 1 cm in size) and superficial which argues against deeper and larger lesions such as aggressive angiomyxoma.

Ok. It's probably a fibroepithelial polyp. So I'm just thinking about our job...we have everyday to ( or want? ) to name by all means all lesions.  But I think sometimes we could just suggest a benign or malignant nature of the lesion ( if possible off course ) and declare our difficulty to make a pretty good diagnosis.

Descriptive report following by - looks reactive. I can't make a specific diagnosis (is always a good line!). Better than saying "non-specific".
