59 year old male - lesions on forehead and back showing similar histology. Biopsy excision on lesion on back ? BCC ? Scar

This one is difficult. This lesion on the forehead made me think in Granuloma faciale, with its mixed inflamatory infiltrate, vasculitis, grenz zone, etc. but than I saw that the patient has the same lesions on the back. It is still possible, but I believe there´s other diagnosis for it.

Prominence of neutrophils over eosinophils in the mixed infiltrate suggests erythema elevatum diutinum.

Yes it is a difficult case and we discussed it at our CPC and the final diagnosis was: extrafacial granuloma faciale
