Hamartoma with combined differentiation. Trichilemmal, trichofollicular, sebaceous, matrix,  and mesenchymal angiofibroma like components. Favour an odd folliculosebaceous cystic hamartoma. 

I thought of calling this nevus sebaceous with different components as you mentioned Vincenzo. FCH tends to be small, papular and located on the face/nose at least that is what I remember

This scalp tumor is a Nevus sebaceous and there are many abnormal adnexial components and well recognized tumors, like a typical Trichilemmoma.

Nevus sebaceus of Jadassohn with epithelial and stromal proliferative changes which probably have occurred during puberty, complicated by late postpubertal arising of a secondary benign adnexal tumor, in this case a tricholemmoma, prompting the patient to seek medical attention.

I called this a desmoplastic trichilemmoma in a nevus sebcaeus.
