Spindle cells in lobules of varying sizes set in myxoid stroma with lobules seperated by fibrous septae. Few bland multinucleate giant vells seen. Lobular architexture and circumscription= Neurothekeoma (Nerve sheath myxoma). DD- Superficial acral fibromyxoma (diffuse and not lobular, + wrong site). Similarly unlikely to be a myxoma. I believe myxoid variant of circumscribed and palisaded neuroma can look similar. However not sure if one can differentiate from a neurothekeoma as both are S100 +ve and an EMA +ve capsule might not be well formed in the former

Agree, Neurothekeoma. Out of interest, would anyone use NK1C3 (CD57) to support this diagnosis?

Isn't NKIC3 for Cellular Neurothekeoma's which are quite different?

Well done everyone. Neurothekeoma is correct. NK1C3 is of help in the diagnosis of cellular neurothekeoma.
