Alopecia areata with granulomatous inflammation

Case 2113 12.7.18 (I H Chaudhry) M 56 occipital scalp, inflamed non-scarring alopecia.

Transverse sections showing “swarm of bees” around bulbs, heavy granulomatous pattern of infiltrate around isthmus. No scarring or loss of sebaceous glands. Increased catagen hairs, small calibre “nanogen” hairs.

DD: syphilis; non-scarring hair loss of acute SLE.
