Salutory case with marked superficial and deep perivascular inflammation, with many cells appearing atypical Granularity must be neutrophilic debris. No consistent IH results to confirm atypical nature. Deeper cut showed straightforward neutrophilic folliculitis.
Diagnosis of bacterial folliculitis or acne-like process supported by the patient’s age and recovery with antibiotics and steroids.
Lesson: “Deeper cuts are the best stain”. Taking sections from the edge of a punch biopsy can give false impression of atypicality. Better to bisect the punch and look at the centre of the biopsy.
