Case 1895  1.9.17 (R. Carr)  M 50 big toe ? ganglion 

Lymphoplasmacytic nodules in a dense fibrous nodule. 

Borrelia PCR -ve (but can be in late stage disease such as this) although serology is usually +ve.  In early Borrelia infection PCR usually +ve and serology can be -ve.
