I think it could be invasive form of superficial spreading melanoma which is characterised by epitheliod atypical melanocytes with pleomorphism and evident mitotic figures yet I liked so much balloon cell melanoma as another DD and I read about it , it really goes a lot with that photo because of the abundant clear cells there looking for the correct answer!

At first look this is a clear cell tumor which appears to infiltrate the dermis. There is a suggestion of pigment which is fine and most likely to be melanin. Based on this and the atypia of the cells I would favour ballon cell melanoma. If the pigment was less conspicious I would consider other clear cell tumors including PECOMA. A very clear and illustrative image Dr McKee.

I like the idea of balloon cell melanoma. Actually I thought of it but I was not sure as the cytoplasm of many of the cells is granular. In Egypt we seldom see melanomas!!! It is so intersting to read and learn from other dermatopathologists all over the world. Great effort Dr. Mckee. Thanks!!
