Good morning all. You are all agreed on the diagnosis. There isn't really any point in not giving the final diagnosis as you are all correct- chondroid syringoma (mixed tumor). It was a primary in the skin. Well done, all of you.

Tubular,branching,lumina , duct like structures suggestive of eccrine differentiation, lined by 2 layers of cells, with marked variation in the size and shape in a basoplilic abundant chondroid stroma.Chondroid syringoma (mixed tumor of the skin. Amazing wondeful picture. Thanks Dr Mckee.

I was thinking in this section all night. Actuall I could not sleep well! Although I did not read about paillary projections in chondroid syringoma, I have to admit that the bluish chondroid and myxoid stroma here is outstanding. I will consider chondroid syringoma but will still think of digital papillary adenocarcinoma as a ddx.

Well, my first impression when I looked at this image was chondroid syringoma due to the bluish stroma that look somewhat chondroid in some areas. However, I can see that the tumor is mainly made of tubuloalveolar and ductal structures, some of them show papillary projections. Also crebriform areas are seen. Esinophilic secretory material is seen in some glandular lumina. I think what I percieved as chondroid syringoma could be areas of dense hyalinized collagen. Thin fibrous septa are seen in the rest of the stroma. I think this could be digital papillary adenocarcinoma.

A nodular and circumscribed lesion with epithelial and myoepithelial cells in abundant chondromyxoid stroma. I think this is a chondroid syringoma. PS: I really like the longer posts with lively discussion on this website.
