Leishmaniasis (again!). I remember Sebastain Lucas used to say he could not tell Leish from Histoplasma capsulatum on H&E in 10% of cases (i.e. the case the day before yesterday). I guess we always need a fungal stain to rule out Histoplasma! You were kind enough to use photographs of a case of Histoplasma (P.973 of your third edition) that I saw when visiting Malawi, Africa. That case had lots of histiocytes (as opposed to macrophages) mixed with plasma cells and lymphocytes but the picture in the book shows the beutiful capsule on the H&E as a distint halo, but I think I took that picture under oil emersion. Nice to relive the case and those wonderful times at St Thomas hospital!

Dear Sir, like you have commented and taught us in case 45, there is dense infiltrate of lymphocytes and plasma cells together with parasitized macrophages. Today this must be Leishmaniasis!! Thank u Sir.
