The correct diagnosis is Spitz nevus. The cytomorphology of the nevus cells is not of concern. I would however agree that on rare occasions, an otherwise typical Spitz nevus can have deep or atypical mitoses or other worrying features and therefore represent a melanoma with Spitzoid features (spitzoid melanoma). It is for this reason that it is very important not to make a confident diagnosis of Spitz nevus if only part of the specimen has been sent for study. I do not recomend use of the term atpical Spitz nevus. It implies diagnostic uncertainty and helps no-one. If you don't know whether it is benign or malignant say so and send the case for a consultation. As the late Ackerman correctly said atypical Spitz nevus must be composed of a mixture of benign and malignant "spitzoid" lesions. I couldn't agree with him more.

Good morning all. I hope that all of you in the UK have a great bank holiday, far away from work. As always, spitzoid lesions cause controversy. It is always difficult to come to a diagnosis when only the most superficial aspect of a melanocytic lesion is available for study. The fact that the Kamino bodies are multiple and aggregated is a helpful clue. Phillip

Just preparing for my finals frcpath! I agree with Donald that these are Kamino bodies but concerned about the cytology and as they are described in melanoma also would suggest this.

Stunning image! The cells look spitzoid to me and so I think this is a spitzoid melanoma. Too cytologically atypical for a spitz nevus.
