The diagnosis is Gout which appears as amorphous material in the dermis with an associated granulomatous reaction. The needle shaped crystals dissolve out in formalin fixed tissue and so fixation in alcohol is required for the preservation of sodium urate monohydrate deposits. Well done to Mo for remembering the earlier spot diagnosis (Case #4); as you say clinical history is important and in this case it occurred at a classical site and showed negative birefringence. Cutaneous oxalosis in comparison show striking yellow or blue birefringence when examined under polarised light. Thank you all for your contributions today. What a gorgeous image! I favour a diagnosis of gout (rather than oxalosis), especially if the specimen was fixed in alcohol.

This is nice case of Gout in contrast to your case in June of Oxalosis, I think although I compared images and could not make a confident differentiation on HE - so need more history please
