Case are uploaded each week day by 10 am UK time with the correct diagnosis will generally be posted at 8 pm UK time. Why not view the most recent spot diagnosis and proffer a diagnosis?

For those who thought that it was a sulphur granule in actinomycosis- well done. If there was any doubt a Gram, PAS or silver stain would resolve the problem. The Spledore Hoeppli phenomenon can be high-lighted with a fibrin stain. Many thanks to all of you who participated. I would be grateful for your help if you would encourage your colleagues to also take part in the spot diagnoses and post answers.

I had a technical error in my internet connection, this is why I am late today. I think I am seeing a wounderful image of sulfur granules surrounded with Splendore Hoeppli phenomenon. This is Actinomycosis, infection by Actinomyces israelii.

I understand (from a little bird) that quite a few folk visit this site but do not post a comment. This may well be because some people are shy or are worried about giving the wrong answer. Please do post comments whenever you look at a case. It really doesn't matter whether your diagnosis is correct or not. The whole point of the spot diagnosis is to give it a try and post a diagnosis and to make it a fun site. The more people who take part, the better the site will become. In any event, we all get diagnoses wrong and I am sure that one of these days, one of you will be able to show me that I have got the diagnosis wrong!! We can all learn from each other.
