I think we have some very smart dermatopathologists on this site; I was unsure about this one and so would go for necrobitoic granuloma dd GA, RN, NL. Dr McKee I have been searching for many sites (such as dermpath india) and the quality of the images is so variable - on this site the quality is perfect, as good as what one sees down the microscope. Thank you for setting this up.

I see epithelioid and foamy histiocytes in addition to angulated giant cells. There is a foci of necrobiosis and cholesterol clefts. That's why I think that this case is about necrobiotic xanthogranuloma.

Can not be epithelioid sarcoma, no atypical epithelioid cells (hyperchromasia, pleomorphism and mitosis). No spindle cells. Besides cholestrol clefts with necrobiosis characterizes NXG.

Agree with comments below (i.e. histiocytic some quite ground glass cells with degeneration and cholesterol clefts IKW the very rare NXG ?paraprotein) but as my paranoia is high and we can't see the whole section or history I would just make sure it is not and epithelioid sarcoma!

I agree also, cholestrol clefts within a necrobiotic collagen surrounded with histiocytes and giant cells. Necrobiotic xanthogranuloma.
