Hyperkeratosis with hypogranulosis, underlying keratinocytes appear normal and not vacuolated, prominent pigementation and pigment granules in stratum corneum, no dermal inflammation, ichthyosis seems very likely, but should it not be associated with moderate hyperkeratosis (that's what the books say not that I have seen a case myself) and this seems rather marked in this field? (of course we are only seeing a small high power field) The pattern of keratinization is somehow reminiscent of trichilemmal, dense hyperkeratin and no granular layer, but would expect a rather undulating junction. There are also lines perpendicular to the epidermis, which together with the abrupt keratinization makes me think about nail histology, which I favour in the end. But I still cannot explain the pigment. This is an excellent website and I have learned a lot looking at all the spot diagnosis.
