Late as usual!! The tumor cells showed focal staining with keratin and EMA. I don't know what other immunohistochemistry was performed. The final diagnosis was monophasic synovial sarcoma.

A haemangiopericytomatous pattern is clearly evident. A more definitive diagnosis is not possible without immunohistochemistry, as there is a broad differential diagnosis to consider. If I have to come up with a spot diagnosis though, I would favour a synovial sarcoma.

I agree with others, haemangiopericytoma-pattern. I would slightly favour solitary fibrous tumour based on patternless cellular arrangement in this field. Saleem

Difficult case - hemangiopericytoma is no longer a specific entity I would believe. Synovial sarcoma I would have expected more spindling of the cells. Note quite sure of diagnosis - requires immuno' - off to bed now!

Haemangiopericytomatous pattern can be seen in solitary fibrous tumour, synovial sarcoma, mesenchymal chondrosarcoma, deep benign fibrous histiocytoma, glomangiopericytoma, haemangiopericytoma (latter rare and diagnosis of exclusion). Looks highly cellular in this field though uniform cytology, I was thinking possibly a synovial sarcoma but needs immuno panel.

With the central ramifyig endothelium lined ramifying vessel and the surrounding tightly packed cells, I am thinking of hemangiopericytoma. Solitary fibrous tumor may show areas of hemangiopericytoid pattern so it could be a differential.
