If this is a biopsy from a neonate, I would favour a diagnosis of subcutaneous fat necrosis of the newborn.

Lobular panniculitis, fat necrosis with some of the cells showing fine esinophilic cytoplasmic strands with needle shaped clefts in between. Inflammatory infiltrate wedged in between the adipocytes. Subcutaneous Fat Necrosis of the Newborn. Perfect picture, Thanks Dr Mckee!!!
