Sorry to be so late but I had a meeting to attend. It is an adenoid cystic carcinoma. The importance is excluding direct extension from a salivary or ceruminous gland tumor and a metastasis. There are occasional accounts of systemic adenoid cystic carcinoma presenting as a skin lesion. So great care is essential before accepting such a tumor as being a primary cutaneous lesion (although the vast majority in the skin are). Have a great weekend. Phillip

Could this not be a metastasis of an adenoid cystic carcinoma ? Also location is very important - if the lesion is around the oral cavity or parotid gland it may be of salivary gland origin and not primary skin

I can see adipocytes at the lower edge of the picture which means that we are in the deep dermis. And I was wondering, if the background is really mucinous, would eccrine mucinous carcinoma be a valid differential in this situation? (And as always one would need to consider a metastasis i.e. from breast in such a tumour depending on the clinical context.) Great case!

I first thought of tubular apocrine adenoma but after deeper looking to the photo I noticed the hyperchromatic appearance of the nuclei being embeded in mucinous stroma I suggest primary adenoid cystic carcinoma

Dr Mckee this is my first visit to the site and I must say I have not seen illustrations of such high standard and also excellent educational cases that I dont see in my day to day practice which is a great learning experience.

I can see nodules of basaloid cells deep in the dermis,with small ducts and large cystic spaces some contain decapitation secretion in hyalinized collagenous stroma so my first impression is nodular hidradenoma
