Well done everyone. The diagnosis is hidradenoma with mucinous metaplasia. This is actually a new case and not the one shown earlier. The problem with single spot diagnoses is that there is obviously a limitation to the number of cases that can be diagnosable on the basis of one single image and as a result over the years there will be repetition. To my mind this is actually advantageous and it shows different examples of entities and also if someone joins our fraternity in the future, they may not have seen the prioir cases. In addition the current online spot diagnoses will not be available forever, as some point we will take down cases probably after around 6 months. One other point, there is really little value in classifying hidraneomas (and many other tumors) as apocrine or eccrine. Over the years, I have gravitated to not really caring whther a tumor is one or the other with obvious exceptions such as hidradenoma papillferum or syringocystadenoma papilliferum which are very clearly showing apocrine differentiation.
