Most of you have got the correct diagnosis. This is a high power view of folliculotropic mycosis fungoides. The lymphocytes are enlarged, hyperchromatic and have irregular contours. There is follicular interface change and infitration of the follicle by the atypical lymphocytes. Clinically, the patient had folliculocentric lesions in addition to plaques. I agree that clinicopathological correlation is critical. Differential diagnoses include pseudolymphomatous folliculitis, follicular lymphomatoid papulosis and at a stretch lichen planopilaris although lymphocyte atypia would not be seen in the last condition. I suppose that a lymphomatoid drug reaction could also be considered. It is important to remember that syringotropic lesions may also be seen in this variant of myosis fungoides. That is a simply stunning image. My first thought was follicular MF/CTCL but agree needs clinical correlation as always.
