This may be insider information but I think this case is the rarer traumatic variant. Most case we see are ganglionic type and have less of an interstitial mucinous pattern and more of a interstitial mucous-filled pseudocyst and more often than not you can see the ganglion wall at the base of the biopsy. I think superficial acral fibromyxoma would be an excellent suggestion in the differential on this field. Regards to all and hope you are already enjoying your weekends

A biopsy from acral skin with a dermal deposition of mucin.In view of the lack of cyst formation I would favour focal mucinosis.

DERMAL-BASED ILL-DEFINED AREA OF MUCINOUS MATERIAL WITHOUT A DEFINITIVE CYSTIC STRUCTRE. THE OVERLYING SKIN APPEARS TO BE DERIVED FROM AN ACRAL SITE WITH HYPERKERATOSIS. IN VIEW OF THIS, MY FAVOURED DIAGNOSIS IS FOCAL MUCINOSIS. NB: I THINK WITH TIME THE LESION MAY PROGRESS TO FORM A DIGITAL MUCOUS CYST.

In acral skin it should be considered a focal cutaneous mucinosis such as digital mucous cyst. There is no evidence of fibroblast proliferation, to think about papular mucinosis. Looks like a biopsy from acral site, propably periungual. Based on the site will favour digital mucous cyst (periungual type), however, apart from the very thick stratum corneum, focal mucinosis will have the same pathological picture as this variant of digital mucous cyst.
