You are all too good. Syringotropic MF is the correct diagnosis. Remember that it often is associated with folliculotropic MF (which is recognised in the new WHO-EORTC as a variant of MF. In some examples the hyperplasia of the sweat glands is mistaken as a granulomatous process. Immunoistochemistry can then be invaluable.

In this section I think I can see syringolymphoid hyperplasia, that's to say: hyperplastic sweat ducts with dense lymphocytic infiltrate around them. This entity might be idiopathic or represent a syringotropic CTCL. In this section I can see that the lymphocytes are atypical with many of them having irregular contours, there is also some plasma cells and esinophils in the infiltrate. So I think this is Syringotropic CTCL with proliferation of the epithelial lining of the sweat ducts.
