This is an interesting case. I am surprised that no-one has suggested pemphigus as a main diagnosis since the predominant feature is that of acantholysis. The differential diagnosis certainly includes Grover's disease and acantholytic acanthoma. Focal acantholytic dyskeratosis could also be considered although there are no corps ronds or grains of Darier. Elsewhere in this specimen (not shown) is a focus showing very typical Darier's disease and that was the clinical diagnosis.
