Dear All! No clues today and so you went down the wrong track! The diagnosis is Anaplastic large cell lymphoma. The atypical cells are seen in the inflammatory cell infiltrate behind the giant cell.

Emperipolisis+ lymphocytes + macrophages +plasma cells = Rasai Dorfman. Emperipolesis + lymphocytes + macrophges = Granulomatous Slack skin.

Dear Dr.Mckee. Many Thanks for the extremely useful website and the interesting case. I am still a novice in dermatopathology and am learning a lot from the cases on this site. My first impression was that of a lymphohistiocytic tumour. The emperiopolesis and large abnormal histiocytes I think is in keeping with cutaneous rosai-dorfman disease.

My first impression is granulomatous slack skin disease. However, the relatively large number of plasma cells and the presence of histiocytes in the right half of the field also showing emperiopolesis (not only the giant cell) will make Rosai Dorfman disease a more likely possibility. Nice Case!!!!

I see a bag filled with lots of markedly enlarged anaplastic cells in the middle of mixed infiltrate of lymphocytes &plasma cells???Are we on the way of lymphoma??Is there any one to give a hand please?
