I think I got it. This is a SCC surrounded by the infiltrate of chronic lymphocytic leukemia/ small lymphocytic lymphoma (B-CLL). This is a common phenomenon with this type of leukemia/lymphoma.

This is an interesting case and is not a lymph node but biopsy of skin. There are two pathologies going on - can you delineate them? Iskander

Hi Marcela, as much as I know and from my very limited knowledge, the epithelial cells in lymphoepithelioma like carcinoma do not show overt signs of squamous differentiation such as horn pearls or dyskeratotic cells. Am I right?? I ruled out the possibility of lymphoepithelioma like carcinoma here because I can see evidence of keratinization.

Islands of atypical squamous cells showing foci of keratinization surrounded with dense lymphocytic infiltrate. My diagnosis is SCC (mostly inflammatory type). Nice case!!
