You are all very good. There are intravascular hyphae (implying fungal septicemia) and therefore the patient is almost certainly immunosuppressed. Although the diagnosis is aspergillosis, fusarium and pseudallescheria enter the differential diagnosis which ultimately depends upon culture. This case was a gift from Nooshin Brinster and it is much appreciated. A wonderful example. I hope that you all have a wonderful weekend and look forward to seeing you on Monday. Phillip

There are large amounts of intravascular fungi, but aditionally I think the stain is colloidal iron and there is mucinous material between collagen fibers. Perhaps a immunosupressed patient with lupus erythematosus victim of intravascular spread of an opportunistic fungal infection.

Septate hyphae invdaing blood vessels. I can see one branching dichotomously in the right half of the field. My diagnosis: Aspergillosis. DDx: Fusariosis.
