The final diagnosis is spindle cell lipoma. As Saleem Taibjee suggested there are no wreath-like giant cells. See you tomorrow.

I think the two components are spindle cells and a vascular componet (multiple small capillary like vessels). If we will also consider the fat as a component of the tumor one would consider: Angiomyofibroblastoma of the vulva or cellular angiofibroma. If the fat in this section is irrelevant, one will consider angiofibroblastoma.

A tough one for a junior like me but I shall dare to attempt a diagnosis anyway. I see a spindle cell tumour infiltrating into the fat lobules. Suspect these are fibroblasts. The fibroblasts infiltrating into the fat is atypical for a histiocytoma and I can't really see any myofibroblasts? Malignant fibrous histiocytoma crossed my mind but the cellular architecture does not look particularly text book typical. DFSP might be a possibility. However on the right hand side the vessel to me looks sick (I might be completely wrong) and there seems to be vessel wall necrosis with what looks like 'dust' surrounding. Therefore I am contemplating a vasculitis as well but can't really tie the two.

Thnk you Dr McKee for the great website. My DD would be fibrous hamartoma of infancy vs lipofibromatosis. The presence of whrly fibromyxoid area is in favour of the first option.
