How easy everyone found this case!! Like your suggestion of Fox Fordyce. We were lucky enough ot see a case when we visited Friedrichschafen last year. There is a nice link to a case on google images: http://archderm.ama-assn.org/content/vol144/issue8/images/medium/dob70046f2.jpg for those interested.

Good morning all. Firstly I must thank Iskander for taking care of the site when I was in the UK visiting my children. There seems to be a general consensus for verruciform xanthoma lead by Marcela. I don't think that anyone will have other diagnoses to add and so will confirm that diagnosis now. I don't think that there is much to add. The nuclei are the wrong shape for Langerhans cell histiocytosis where they should be reniform (on a good day) and often a longitudinal groove is present. Furthermore they are generally very epidermotropic. I am not sure where secondary syphilis fits in. There really is nothing to suggest that diagnosis.

I would have 3 differentials namely, verucciform xanthoma, langerhans histocytosis and secondary syphillis

Could this be Langerhan's cell histiocytosis
