Obviously some of you have a better memory than I have!! Yes the diagnosis is leprmatous leprosy. The presence of a few lymphocytes and one or two plasma cells are clues to the diagnosis. I don't see a well formed granuloma. It is always a diagnosis to bear in mind when looking at a perivascular xanthoma cell-like infiltrate. Atypical mycobacterial infections can sometimes look similar.

Granuloma with few plasma cells and almost no lymphocytes: it could be syphilis, tuberculoid leprosy, even sarcoidosis.. I would definitely do stains and immunoperoxidase for Treponema pallidum.

Dear Dr McKee,I have a question about this case , previously diagnosed as lepromatous leprosy (case no 11).As far as I know (from my little informations) that in lepromatous leprosy, no activation of macrophages to form granuloma and i can see a granuloma formation here in this photo (if i am not wrong). Is there an explanation for this? Thanks a lot sir. Its a marvellous chance to be learned by you.
