It is a binucleate plasma cell. The nuclei are too small for an RS cell - much larger and amphophilic nucleoli and and more oval nuclei with vesicular chromatin. You can see the peri-nuclear hoff and the purple cytoplasm (amphophilic) of the plasma cells that have a small central dark chromatin dot and the peripheral aggregates of chromatin (like a clock face). It is a nice cell but has no meaning in this case which appears to be a case of LCH. REgards to all, Richard

Langerhans cell histiocytosis. Is that cell at the left lower corner owel eyes cell(Reed-Sternberg cell)? Could then add DD of mixes cellularity Hodjkin's Lymphoma? Or its just a multinucleated giant cell in the context of LGH?

Large histiocytes, most of them with indented reniform nuclei and lots of esinophils. Esinophilic Granuloma (one of the 3 types of Langerhans Cell Histiocytosis, now known collectively as Langerhans Cell Disease)

Forgot to mention one thing. Having said that this is is LCG, these atypical changes still bother me. So malignant hisiocytosis should be differetiated, although I have never seen one before.

It looks so much like eosinophilic granuloma. The presence of atypical mitosis and pleomorphism of the histiocytes may be just reactive. So I agree this is Langerhans cell histiocytosis.
