Interesting discussion. The actual diagnosis is basaloid cells in a pilomatrixoma with lots of mitoses!! Goes to show that it is not all that easy at high power. Rodrigo you deserve an award - thank you to the rest for your differentials.

Undifferentiated SCC, Or Large cell lymphoma. I think the cells are large for MCC. Immunohistochemistry is mandatory.

Large cell malignant neoplasm. I suspect either an undifferentiated carcinoma, a Merkel cell carcinoma or a large cell non-Hodgkin lymphoma.
