Yet again spectabular image. Nice "ropey" (presume that derives from old fashioned coiled ropes) collagen (and all the other features) for a spindle cell lipoma (these lesions are often very myxoid).

As I said before, I am trying to learn dermatopathology. I recogized these spindle cell proliferation from the start and almost no lymphocytes ( old lupus panniculitis could have sparse inflammatory cells but have myxoid stroma and hyaline deposition). I was wrong. I agree with spindle cell lipoma .

Spindle cell lipoma is nice idea but I think the spindle cell proliferation is less than needed to diagnose spindle cell lipoma and also the myxoid stroma in this section is somehow abundant. Thought of superficial acral fibromyxoma (loosly and randomly arranged spindle cells, myxoid and collagenous stroma and mature fat cells which have been reported in some cases of superficial acral fibromyxoma). Less likely I will consider fibrous hamartoma of infancy. I might be wrong anyway!! Nice case, anxious to know the correct diagnosis.
