Well done. Pemphigus foliaceus is the correct answer although all of the differentials are appropriate and would have to be excluded. Have a wonderful Christmas. Phillip

Good Morning everybody. First of all, I would like to wish u all a Merry Christmas, happy holidays and a happy new year. I agree with the detailed description of my colleague Eman. Due to the evident beautifully demonstrated rounded acantholytic cells, I also favour superficial pemphigus (P. Foliaceus or P. Erythematosus). SCPD and IgA pemphigus (subcorneal type) usually show occassional or mild acantholysis but still they are important DDx here.

Eman El-Nabarawy - Cairo University (Egypt) Wrote:

Subcorneal blister filled with mainly neutrophils with good number of acantholytic cells and dermal infiltrate of mainly neutrophils, few eosinophils. Favor superficial pemphigus. DD subcorneal pustular dermatosis and IgA pemphigus (subcorneal type) for clinicopathological correlation.
