The diagnosis is Aneurysmal fibrous histiocytoma. I take your point about pigmented DFSP which is naturally a differential but the pigment looks more golden and clumped in image 3 reflected that it is haemosidrin.

Aneurysmal fibrous histiocytoma may show also focal extension to SC fat like in this case, but I have never seen in aneurysmal dermatofibromas the storiforme closed pattern with a radius of gyration so short wich is often seen in dermatofibrosarcomas. On the other hand the abundant pigment is brilliant like hemosiderin, I have not seen melanin in the images, so I bet for a rare aneurysmal fibrous histiocytoma and not for a Bednar tumour (pigmented DFSP).

My first impression was pigmented DFSP but after careful inspection I realiazed that there is no pigmented dendritic cells and there are many cleft like and rounded spaces, some on the right half of the first image contain RBC's so I am in favour that this pigment is hemosiderin and I am in favor of a hemosiderotic benign fibrous histiocytoma or aneurysmal fibrous histiocytoma especially that the spaces do not have obvious endothelial cells, the spaces are not very large or cavernous but it is said that they may vary from cleft like to cavernous.. Aneurysmal fibrous histiocytoma may show also focal extension to SC fat.. Interesting case!!!!!!!!

BEDNAR TUMOUR (PIGMENTED DFSP). THE DIFFERENTIAL DIAGNOSIS INCLUDES ANGIOMATOID BFH. WOULD LIKE CD34 AND FXIIIa TO CONFIRM.
