Lichen sclerosus is correct. Post radiation effect is an excellent differential diagnosis. However, there was no history of prior radiation in this patient.

I would agree in that the features are in keeping with Lichen Sclerosis. However, considering the site, is there a history of irradiation ? Could the features be representative of a chronic radiation dermatitis ? : hyperkeratosis, irregular epidermal hyperplasia, basal vacuolar damage, dermal hyalinisation, dermal chronic inflammation, telangectasia, . I spotted a few odd-looking fibroblasts in the left-hand corner of the highest power image.Z
