Marginal zone lymphoma is the correct diagnosis. The immunohistochemistry was in keeping with this diagnosis although I cannot remember the exact details. This may have been a case of Richard Carr's. If it was, perhaps he can add some information. Well done everybody.

THERE IS A DIFFUSE DYSCOHESIVE SHEET-LIKE PROLIFERATION OF CELLS, ADMIXED WITH PLASMA CELLS, HISTIOCYTES AND EOSINOPHILS. SOME OF THE LESIONAL CELLS HAVE A DISTINCT PLASMACYTOID MORPHOLOGICAL APPEARANCE. I FAVOUR A LOW GARDE HEAMATOLYMPHOID NEOPLASM, SUCH AS LYMPHOPLASMABLASTIC LYMPHOMA (LPL) OR MARGINAL ZONE LYMPHOMA (MZL). HOWEVER, A PLASMACYTOMA IS ALSO REQUIRES POSSIBLE CONSIDERATION. IMMUNOHISTOCHEMISTRY IS DEFINITIVE. NOTE: I THINK THE PATIENT IS A LITTLE YOUNG FOR BOTH LPL AND PLASMACYTOMA!!!

A dense diffuse dermal polymorphic infiltrate composed of plasma cells, histiocytes and small to medium sized lymphoid cells with inconspicuous nucleoli with numerous small vascular proliferation. The features are of a low grade lymphoid neoplasm, suggestive of either lymphoplasmacytic lymphoma or marginal zone lymphoma. Immunohistochemistry is required to confirm.
