first diagnosis...Reticulohistiocytoma... multinucleated cell with eosinophilic and groundglass cytoplasm...and emperipolesis has been reported. 2nd consideration...Necrobiosis lipoidica.

This is difficult case of granulomatous inflammation and on this one image there are many differentials, however, the actual diagnosis was metastatic Crohn's disease. Better luck tomorrow.

I think the lymphocytes have irregular contours (atypical). Together with the histiocytes, plasma cells, esinophils and MNGC showing emperipolesis, my first impression is granulomatous slack skin disease.

The picture shows granulomas with multinucleated giant cells plasma cells and scattered eosinophils, there is a hint of emperipolesis.... I would think of cutaneous Rosai Dorfman vs lepra
