This is a case where clinicopathological correlation and immunohistochemistry are essential. The differential diagnoses you have all come to are good. They include pagetoid reticulosis, CD8+ epidermotropic T-cell lymphoma, mycosis fungoides showing marked epidermotropism, an epidermotropic T-cell pseudolymphoma etc. I have seen a couple of epidermotropic Merkel cell carcinomas so that could be within the differential diagnosis. I don't think Ketron Goodman disease exists. It's description antedated the era of immmunohistochemistry and it is most likely to represent an extremely epidermotropic example of MF or a CD8+ epidermotropic T-cell lymphoma. This was a case of pagetoid reticulosis. Have a great weekend.

Hyperkeratosis, parakeratosis and psoriasiform hyperplasia. Epidermis shows marked epidermotropism with large atypical lymphocytes mainly in the lower third of the epidermis. Some forming clusters (Pautrier microabscesses).. I bet this is Pagetoid reticulosis (Woringer Kolopp).. Lovely!!
