Well done everyone. Plaque stage Kaposi's sarcoma is the correct answer. The lack of endothelial cell atypia, multilayering etc excludes angiosarcoma. The specimen came from a known HIV/AIDS patient. I don't know whether HHV8 immunohistochemistry was performed.

MALIGNANT VASCULAR NEOPLASM - FAVOUR ANAPLASTIC KAPOSI SARCOMA; HOWEVER, ANGIOSARCOMA CANNOT BE EXCLUDED. I WOULD LIKE TO PERFORM IMMUNOHISTOCHEMICAL STAINS TO DIFFERENTIATE BETWEEN THE TWO - HHV8 AND CD31 / CD34.

Vascular proliferation, spindled cells, extravasataed RBC's and inflammatory infiltrate (lymphocytes and plasma cells). I can also c esinophilic globules. I am thinking of Kaposi Sarcoma.
