This case was kindly shared with me by Nooshin Brinster, Virginia. It comes from a lady who had radiotherapy for Ca breat. It was diagnosed as atypical vascular proliferation. The differential diagnoses given in the comments are very good.

My differentials include: hobnail haemangioma or atypical postradiation vascular proliferation. Need a full history.

Wedge shaped proliferation of vascular channels lined by hobnail endothelial cells. Few papillary projections into lumen.Would like to see what perl's shows. However looks like a hobnail haemangioma. DD includes well differentiated Angiosarcoma. However do not see multilayering or significant atypia. KS is also in the diff., but does not look like it. Given the site, I did wonder about Atypical vascular proliferation following radiotherapy. However the vessels there are lymphatic unlike in this picture.
