The differential diagnosis of small blue cell tumors is always difficult and depends upon previous history or immunohistochemistry. This case is a metastatic bronchial neuroendocrine tumor. Confirmed by both of the above. I will be away all of the rest of this week and Iskander will be looking afer the spot diagnoses

Dense nodular infiltrate composed of small lymphocytes with areas showing paler centres made up of large lymphocytes with vesicular nuclei. I think this could be chronic lymphocytic leukemia/ small lymphocytic lymphoma. The larger cells in the centres may be pro-lymphocytes or paraimmunoblasts. It might be also transforming to large BCL. Definitly extensive immunohistochemistry is needed.

HIGH GRADE MALIGNANT NEOPLASM. I FAVOUR A LARGE CELL HIGH GRADE NON HODGKIN LYMPHOMA. THE DIFFERENTIAL DIAGNOSIS INCLUDES: (1) AMELANOTIC MELANOMA; (2) POORLY DIFFERENTIATED METASTATIC CARCINOMA. I WOULD NEED IMMUNOHISTOCHEMICAL STAINS TO DIFFERENTIATE.
