This is a protypic acral nevus with only mild atypia. The cellular dyscohesion in the nest is characteristic. Dendritic processes are present another feature. The severe atypia and marked pagetoid spread of "maniac" are not present.

Compound acral naevus with mild atypia (cytological and architectural) consistent with naevus of special site. It appears symmetrical, no pagetoid spread, atypia minimal, lack of rete ridge elongation and abscence of dermal fibroplasia.
