Well done everyone. Spiradenocarcinoma is correct. The upper two images show a pre-existent telangiectatic spiradenoma. I don't recognize a category of mitotically active spiradenoma.

My first impression was telangiectatic spiradenoma, After reading the comments and revising the images, I found several mitotic figures, they look typical, The cells do not look pleomorphic, I favor mitotically active spiradenoma. The clinical picture of spiradenocarcinoma is that of a long lasting lesion that underwent sudden enlargement. Nice case!,,

I would have been happy to call it a mitotoically active spiradenoma based on this morphology. I know I am a lot less malignant than some on adnexal tumours. Probably because I have not seen enough cases going bad!!
